Patient's Physician--
Information, including documenting photographs, are submitted to patient's
physician (or nurse practitioner or physician's assistant) on a regular
basis. They are a partner in the treatment team. No
physician? That is not a problem. Many of our patients are
referred by other long-term care facilities, other medical professionals, their
families or self-referrals.
