Patient's Physician
Information, including documenting photographs, is submitted to patient's physician (or nurse practitioner or physician's assistant) on a regular basis. They are a partner in the treatment team. No physician? That is not a problem. Other long-term care facilities, medical professionals, patients families or self-referrals refer many of our patients.
